Submission of this form indicates that you have an appropriate need to share CUI and your agreements that you will share such CUI with AvePoint only through our designated CMMC-compliant environment. Your submission of this form alone does not permit your organization to share CUI with AvePoint until this form is submitted, approved, and you have been provided notice that your organization has been migrated to our designated CMMC-compliant environment.

Your organization holds Controlled Unclassified Information (CUI) and has an appropriate need to share that CUI with AvePoint.
Any CUI shared with AvePoint will be transmitted only through AvePoint's CMMC Level 2 compliant environment, following approval and migration of an account.
You will not send CUI to AvePoint through non-approved channels or systems.
AvePoint may use the information provided in this form solely for the purpose of evaluating, onboarding, and managing CMMC-scoped engagements.
Participation in the CMMC-compliant environment is governed by applicable contractual terms and compliance requirements.
